Provides Case Management services to persons age 16 and older who meet the definition of being Developmentally Disabled, as defined by the State of Montana. Case Management services will be provided to assist these persons to again access to needed medical, social, educational, vocational, recreational, and other services.
PRINCIPAL DUTIES AND RESPONSIBILITY:
Established a customer caseload with Program Director. Provides assistance in accessing and obtaining needed services. Provides coordination and the comprehensive development of an Individual Service Plan and monitors that plan as required by established rules. Provides crisis intervention for people on the caseload in a personal, social or medical crisis who require assistance within 24 hours. Prepares reports applications, and supporting documentation as requested by the supervisor within timelines established by Program Director.
KNOWLEDGE, SKILLS AND ABILITIES:
This position requires a Bachelor’s Degree in Human Services, Social Work, or related field. The employee will also be required to undergo training and to pass testing in First Aid and CPR, Medication Certification, Bloodborne Pathogens, Fire Safety, and HELP; additional training may also be required. Requires a Montana Driver’s License and clean motor vehicle record. Requires computer literacy skills, including the ability to navigate, record, and overall basic computer skills needed for performance-based software as well as an electronic medical records system.
The successful applicant may be eligible to apply for a Tier II pay rate of $15.75/hour after six months of employment and in addition to supplying qualifying materials.
