Case Management

AWARE’s case management provides assistance for adults and youth with disabilities in accessing, referring and coordinating services and supports that allow each individual to live at the highest quality of life. Case managers meet with individuals to assess and develop goals based on the individual’s needs. They also provide assistance to the individual in order to access services or supports. These services enhance an individual’s independence and interdependence with others in the community, expand options for participating in – and contributing to – society, increase an individual’s skills and abilities, and assist individuals to live in the community. Aspects of case management include, but are not limited to, assistance in accessing social, leisure and recreational supports; health, health maintenance, and health safety supports; and cultural and educational services.
